Sperm morphology is the shape and size of sperm cells, and is assessed during a semen analysis: Normal sperm: Has a smooth oval head, a well-defined cap, and a long tail Abnormal sperm: Has a large or misshapen head, a crooked or double tail, or other defects. Sperm morphology is important because it can affect a sperm's ability to fertilize an egg. However, it's not the only factor that contributes to male fertility, and having a large percentage of abnormally shaped sperm is normal. In fact, most male fertility experts agree that sperm morphology is a poor predictor of infertility unless nearly 100% of the sperm are abnormal. Factors that can lead to abnormally shaped sperm include: increased testicular temperature, exposure to toxic chemicals, infection, and genetic traits. Sperm morphology is evaluated using a variety of criteria, including the Kruger Strict Criteria and the World Health Organization guidelines. The Kruger Strict Criteria is considered the most clinically useful morphology criteria. Sperm morphology results are reported as the percentage of sperm that appear normal when semen is viewed under a microscope. The precise range can vary, but typically a normal or healthy sperm morphology range is between 4 and 14 percent NF. A score below 4 percent may mean it takes longer than normal to achieve pregnancy.
